We are glad you are considering our office for your dental
needs. We will do everything possible to make sure your time
spent in our office is a pleasant experience. To alleviate
the rush of doing paperwork before your appointment you can
print out your new
patient forms here (Note: forms will open in new
browser). Complete them at your convenience at home then
bring them with you to your first appointment. If you choose
to wait until you get to our office, please arrive 15
minutes prior to your appointment time to fill out
paperwork.
Insurance Cards
Please bring your insurance information with you to your
appointment. An insurance card is preferred but not
necessary. As long as you have the name and social security
number of the insured, the name and phone number of the
insurance carrier and the group number, we will be able to
contact your insurance company and get the information we
need to file your claim. As you can see, we want to make
this experience as easy as possible for you and your family! |
